Parking Citation Review Form - here
In order to contest a parking citation please fill out the Parking Citation Review form and return it to our office along with a copy of your citation.  The Traffic Sergeant will review your explanation and a letter will follow to advise whether your citation is valid or dismissed.
 
Block Party Application Form - here
Is your neighborhood planning a block party?  If so, please fill out the Block Party Application and submit the form to the Records Division a minimum of 10 business days before your event.  A fee of $105.00 made payable to the City of Los Altos is due upon pickup.
 
Ride Along/Sit Along Form - here
If you are interested in riding along with a Police Officer or a sit along with a 9-1-1 Dispatcher please fill out the Ride-Along/Sit-Along form.
 
Alcohol Permit Application - here
If you are planning on hosting a gathering on city property where alcohol will be consumed, please fill out the Alcohol Permit Application and submit the form to the Recreation Department (if renting a city owned property) or to the Police Department if gathering at a park.  The fee for an Alcohol Permit is: $65. 
 
Police Report Copy Request - here
If you need to request a copy of your police report just fill out the Police Report Request form.  There is no fee for a copy of a report.  Please allow up to 10 days once we have received your request and someone will call you when the copy is ready or if more information is needed.
 
Request Code Enforcement Online - here
You must be a resident of the City of Los Altos, California and the code violation must have occurred within the City of Los Altos. Residents of surrounding communities should contact their local police departments.

The code violation must not require immediate attention.


If the code violation represents an emergency, requires an immediate response, or causes a danger to the public, dial 9-1-1 immediately. The online form is for non-emergency use only
 
Report a Traffic Problem Online - here
If you have an emergency, dial 9-1-1. This online form is provided as a convenience for non-urgent issues. Use this form to report recurring problems, request a traffic trailer in your neighborhood, or to contact the Traffic Department with a non-emergency request.
 
Court-Ordered Visitation Violation Report - here
The District Attorney's Office has implemented a self-reporting system, for use by Santa Clara County residents only, to report violations of court ordered visitation.

Parking Permit Application - here
The Downtown Parking Permit Program provides all day parking for downtown business owners and employees. By displaying a valid parking permit on your car, you can park all day without the worry of getting a parking ticket or moving your car from space to space. Using a parking permit is an easy way to park all day while working in Downtown Los Altos.

The Sidewalk Application Event form is for merchants  downtown to fill out when they are planning to sell their goods in front of their store when there is 10 or more businesses, a merchant association or a community sponsored event.

Clearance Letter Application - here
If you need to obtain a local records clearance from the City of Los Altos please fill out the Clearance Letter application. To submit your application it must be in person along with a government issued photo identification and payment. If you are unable to submit your application in person you must have your identity verified by a Notory Public. The original notary, including your photo must accompany your application, payment and a prepaid self-addressed stamped envelope for your Clearance Letter to be sent back. The fee is: $31.00 and the check made payable to “City of Los Altos”.

The Vacation Contact Form is information provided to the police department on a voluntary basis by the home owner.  The information regarding your property is kept confidential and if the need arises will aid our staff in contacting you or your designee.  Please read the highlighted portion at the bottom of the "Vacation Contact Form" for more information. 
 
Solicitor Permit Information - here
In order to lawfully solicit door-to-door in the city of Los Altos you must be permitted by the Los Altos Police Department.  All the information you will need to determine if you or your company is required or exempt according to our municipal ordinance can be found in the attached documents.  Once the application is filled out please sign and date the forms prior to turning it into our office.
